Information shared on this website should not be taken as as a replacement for medical advice. Please consult your physician or healthcare provider before enacting any new health, nutrition or exercise change.

Release

I understand that by requesting a customized nutrition or personal training program, purchasing an e-book, or engaging in a contest preparation, I must complete a preliminary client intake form that is provided by Revive Wellness, Allison Fahrenbach. I understand that I must accept full responsibility for my health and well being in participating voluntarily in any new or modified exercise, fitness, or nutrition program. I participate in the activity at my sole risk and responsibility; I release, indemnify and hold harmless Revive Wellness, its servants, agents, employees and representatives from and against all and any actions or claims which may be made by me or on behalf or by other parties for or in respect of or rising out of injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever. I also agree that in the event that I am injured or suffer damage, I will bring no claim, legal or otherwise against Revive Wellness in respect of that injury or damage.

Payment & Refund Policy

All Payments and purchases for services, programs, consults, and packages are non- refundable.  All programs and purchases must be redeemed/initiated within 15 days of the purchase date. Failure to redeem/initiate your coaching within 15 days of program purchase will result in a loss of total program time. Full payment of coaching programs must be made according to the payment agreement established between you and Revive Wellness. If adherence to the legal payment plan terms is not met at the behest of the client, Revive Wellness reserves the right to exercise any and all authority under the law to extract payment due from the client. ​All recurring monthly payment plans require a 30 day written cancellation notice. This is non-negotiable. Services purchased but not utilized will not be refunded.

Right to refuse service

Allison Fahrenbach/Revive Wellness reserves the right to refuse services to any potential client for any reason, at any time.
