Release
By requesting a consultation or enrolling in coaching with Revive Wellness, Allison Fahrenbach, I acknowledge and agree to the following:

Accuracy of Information
I understand that I am required to complete a preliminary client intake form prior to beginning any consultation or coaching services. I affirm that all information provided on that form is accurate and complete to the best of my knowledge. I acknowledge that any omission or inaccuracy may affect the quality or safety of recommendations provided. I release Revive Wellness and Allison Fahrenbach from any liability arising from inaccurate or incomplete information that I provide. -
Assumption of Risk
I acknowledge that participation in any new or modified exercise, nutrition, or lifestyle program involves inherent risks. I voluntarily choose to participate and accept full responsibility for my health, wellbeing, and any outcomes that result from my participation. -
Release of Liability
I hereby release, indemnify, and hold harmless Revive Wellness, Allison Fahrenbach, and any affiliated staff, agents, or representatives from all claims, demands, or causes of action arising from injury, illness, loss, or damage that may occur to myself or my property as a result of my participation—whether caused by negligence, breach of contract, or otherwise. -
No Claims
In the event of injury, illness, or damage, I agree that I will not pursue any claim, legal or otherwise, against Revive Wellness, Allison Fahrenbach, or their representatives.
Payment, Cancellation & Refund Policy
All payments and purchases for services, programs, consultations, and packages through Revive Wellness are non-refundable.
Program Redemption:
All coaching programs and services must be redeemed or initiated within 15 days of the purchase date. Failure to begin services within this timeframe will result in forfeiture of program time without refund or credit.
Payment Terms:
Full payment for coaching programs must be made according to the payment agreement established between the client and Revive Wellness. If the client fails to adhere to the agreed-upon payment schedule, Revive Wellness reserves the right to pursue any and all legal remedies available to recover the balance owed.
Program Cancellations:
No coaching program may be canceled within the first six (6) months of enrollment. After the initial six-month period, all recurring monthly payment plans require a minimum 30-day written notice to cancel. This policy is non-negotiable.
Subscription Services:
For any ongoing subscription or membership-based services, a full 30 days’ notice prior to the next billing date is required to cancel billing. Cancellations made less than 30 days before the next billing date will take effect in the following billing cycle.
Unused Services:
Services or sessions purchased but not utilized will not be refunded, credited, or transferred.
By enrolling in any service or program, you acknowledge and agree to these terms in full.
Right to refuse service
Allison Fahrenbach/Revive Wellness reserves the right to refuse services, to any potential client for any reason, at any time. Allison Fahrenbach/Revive Wellness reserves the right to cancel a coaching contract with any client, for any reason, at any time.
